A Microsoft 365 campaign that used an adversary-in-the-middle (AitM) relay to capture live sign-in sessions has compromised 97 accounts across 79 organisations, according to Huntress research reported on 27 October by The Hacker News.

Huntress tracked the activity between 14 July and 22 September 2026 and logged 497 successful Microsoft 365 sign-in events tied to the campaign. The volume points to a broad, opportunistic sweep rather than a surgical strike: repeated returns to the same harvested sessions, used for mailbox reconnaissance and credential harvesting, across dozens of tenants over roughly ten weeks.

How the relay beats MFA

The mechanics are why this matters to anyone running M365. In an AitM attack, the phishing proxy sits between the victim and the real identity provider. The user completes a genuine login — password plus MFA challenge — and the identity provider, satisfied that every factor checked out, issues a session token. The relay forwards the authentication and keeps the token.

That token is a bearer credential: whoever holds it holds the session. In general, such tokens can grant access to mailboxes and, depending on scope, other services. The token carries the trust earned by the legitimate MFA-satisfied login, but it is not bound to the device, browser, or person who originally satisfied the challenge. An attacker replaying it elsewhere inherits access without touching a password or second factor again.

The practical consequence: MFA protects the act of authentication, not the session that authentication produces. Signing in once, safely, does not make the resulting token safe to hold — and a control that only runs at sign-in will not fire again when that token is replayed from a different machine days later.

What IT admins can do

For administrators running Microsoft 365 in Hong Kong, the incident points to a specific set of hardening steps rather than a generic "enable MFA" message:

  • Revoke sessions, not just passwords. Suspected token theft requires a full sign-out of refresh tokens across the tenant; password resets alone leave the stolen session valid.
  • Enable Continuous Access Evaluation (CAE). CAE lets Entra ID re-check conditions during a session and cut access within minutes when risk signals change, instead of waiting for token expiry.
  • Require compliant devices in Conditional Access. Binding sign-in to managed, health-checked devices narrows where a replayed token can be used.
  • Move toward FIDO2 / passwordless. Device-bound passkeys are not replayable in the same way as session cookies.
  • Audit sign-in logs for unfamiliar IP addresses, legacy authentication patterns, and tokens redeemed outside expected device contexts.

An intrusion that leaves little trace

The campaign generated very little detectable signal: valid tokens, valid sessions, legitimate-looking sign-ins. Detection has to come from context — device posture, geography, IP reputation, session lifetime, refresh-token redemption point — rather than from the authentication event itself.

The finding should also deflate one common comfort: organisations that treat MFA as a completed phishing defence are relying on a control that, by design, finishes before the attacker ever uses the token. The 79 affected tenants illustrate that those relying on MFA alone, without phishing-resistant or session-bound controls, are exactly the targets this technique seeks.
